What is ASCVD risk? ASCVD (atherosclerotic cardiovascular disease) means disease caused by fatty plaque building up in artery walls: heart attack, death from coronary heart disease, and stroke. "10-year ASCVD risk" is an estimate of the chance that a person of a given age, sex, cholesterol, blood pressure, smoking and diabetes status has a first such event within 10 years. It is a population estimate, not a prediction for one person.
What this tool does. Apex ASCVD Risk Calculator works out the 2013 ACC/AHA Pooled Cohort Equations from values you supply (Goff DC Jr et al. 2013 ACC/AHA Guideline on the Assessment of Cardiovascular Risk. Circulation 2014;129(25 Suppl 2):S49-S73. DOI 10.1161/01.cir.0000437741.48606.98 (Appendix 7, Table A)) and shows the 10-year percentage and the guideline band (low under 5%, borderline 5 to 7.4%, intermediate 7.5 to 19.9%, high 20% or more).
Limits. These equations are superseded: the 2026 ACC/AHA dyslipidemia guideline uses the AHA PREVENT equations. The Pooled Cohort Equations are race-based and overestimate risk in contemporary cohorts, so this tool is for education and comparison only.
